About the Organization

Community Physical Therapy & Associates (CPT) provides rehabilitation services in inpatient and outpatient settings to individuals in the greater Chicago metropolitan area. Our board-certified therapists offer Physical, Occupational and Speech Therapies in post-acute rehabilitation and skilled nursing facilities, in the home setting and at outpatient locations.

Our goal is to help individuals achieve optimal clinical outcomes following an illness, injury or hospitalization. Our team will make sure you receive the right therapy for your individual needs.

CPT is a member of the American Physical Therapy Association, the Illinois Physical Therapy Association and the National Association for Home Care. This helps us keep current with best practices in rehabilitation and helps us set standards for the best in rehabilitation care.

We are seeking a Lead Physical Therapist in our Flanagan, IL location. This role is full-time.

Qualifications

  • Graduate of physical therapy school program approved by the Council of Medical Education of the AMA in collaboration with the APTA or its equivalent.
  • Must have five years of practical physical therapy experience and applicable lead experience OR any combination of training and experience.
  • Must have the ability to lead staff of clinicians.
  • Licensed, certified, or registered to practice in the State of Illinois as a physical therapist
  • Functions according to the APTA Code of Ethics and Standards of Practice
  • Must speak, write, read and comprehend the English language
  • Must be physically able to meet the demands of the position
  • Ensure that credentials that are required to CPT/Facility are current according to CPT’s policies and procedures
  • Must have the ability to work independently and make decisions when circumstances warrant such actions
  • Must have the ability to effectively communicate with patients, family, staff, visitors and physicians.
