About the Organization

Community Physical Therapy & Associates (CPT) provides rehabilitation services in inpatient and outpatient settings to individuals in the greater Chicago metropolitan area. Our board-certified therapists offer Physical, Occupational and Speech Therapies in post-acute rehabilitation and skilled nursing facilities, in the home setting and at outpatient locations.

Our goal is to help individuals achieve optimal clinical outcomes following an illness, injury or hospitalization. Our team will make sure you receive the right therapy for your individual needs.

CPT is a member of the American Physical Therapy Association, the Illinois Physical Therapy Association and the National Association for Home Care. This helps us keep current with best practices in rehabilitation and helps us set standards for the best in rehabilitation care.

We are seeking a Speech Therapist in our Franklin Grove, IL location. This role is full-time.

  • Master’s degree in communication disorders
  • Must meet the requirements for State of Illinois Department of Professional Regulation
  • Must possess the Certificate of Clinical Competence (CCC) of the American Speech and Hearing Association (ASHA) or actively completing the certification in clinical fellowship year (CFY).
  • Must speak, write, read, and comprehend the English language
  • Must possess current CPR certification and maintain it yearly
  • Must possess OSHA certification and maintain it yearly
  • Must have TB Test and Hepatitis Vaccination or waiver
  • Must have the ability to work independently and make decisions when circumstances warrant such actions
  • Must have the ability to effectively communicate with patients, family, staff, visitors and physicians.
  • Rehab Optima experience is preferred
